Step

Based upon
the findings of the completed Assessment and Evaluation process, A Senior
Solution will then develop an Individualized Plan of Care.
The Plan of
Care is a written document summarizing the Assessment and Evaluation results as
well as making recommendations for the best strategy and course of action
available to the client.
An
Individual Plan of Care includes:

Written
documentation summarizing the initial Assessment and Evaluation.

Recommendations
for home modifications, specific adaptive equipment and purchasing
assistance.

Recommendations
for alternative living arrangements.

Assistance
with required documentation for assisted living or nursing home placement
and collaboration with primary physician.

Recommendation
and referral for housekeeping, companionship, transportation, home care and
personal care providers.
Step
Three: Follow up Services

Ongoing
monitoring, reporting and development of the Plan of Care, as requested by
family
