Please note: Due to the
price of gasoline, we now charge a $5.00 mileage fee (if outside of 20 miles) or
$10.00 mileage fee (if outside of 50 miles) to our Assessments. We will
discontinue this fee, once gasoline prices are below $3.00 per gallon. We

One:
If you are
looking into the options for long term care and placement, A Senior Solution
is there to provide you with full assistance. We begin with an initial
Client Assessment and Evaluation process. The Assessment and Evaluation
process incorporates our understanding that each client is unique with a
different set of needs.
Our
Assessment and evaluation services include:

A
medical history review, obtaining medical records, communication with health
care team and personal physician.

Alzheimer's
Disease assessment and staging of disease process.

An
evaluation of Activities of Daily Living (ADLs): bathing, dressing,
toileting, transferring, and eating are evaluated to determine level of
independence versus dependence.

Evaluation
of Instrumental Activities of Daily Living (IADLs). Ability to take
medications, communicate, perform routine tasks such as food preparation,
banking, laundry, shopping.

Home
safety evaluation, fall risk assessment, accessibility of pathways,
lighting, handrails, bathroom and overall safety.

Psychosocial
assessment, social interactions and Depression Scale testing.
